Infection, Inflammation, deposition, or malignancy can cause panniculitis.

T cell lymphoma is a skin manifestation based malignancy.
Pancreatitis and AAT deficiency also can cause panniculitis.
Yes biopsy is required.
But a thorough evaluation for above causes also required.
