Greetings,
I have chronic sinus problem. Last couple of days I have a headache and I feel little bit dizzy. I think my sinuses don't drain. What are my options. I'm using neti pot and fluticosane nasal spray as my doctor recommended with little relief. I'm breastfeeding. Thanks..

Welcome, as an Internist I will try to answer your Medical Question, for informational purposes only, please see your doctor as needed.

neuromd2012 :

Hello are you there?

Customer:

yes

neuromd2012 :

as you are breast feeding,

neuromd2012 :

some of the medications are out

neuromd2012 :

like guaifenesin (mucinex)

neuromd2012 :

it is a category C, and the company suggests not to use it

neuromd2012 :

so have you seen an ENT doc?

neuromd2012 :

have you had an endoscopy?

neuromd2012 :

could you have a polyp? or mucocele?

Customer:

yes, i saw my primary care doctor about a month ago. I was feeling my ear clogged and it turned out it was because of my sinus problem. He prescribed nasal spray and advised me to continue using neti pot

neuromd2012 :

right

neuromd2012 :

that all makes sense

neuromd2012 :

since you are breast feeding your blood volume remains high, and you can get congested easier

neuromd2012 :

but you don't want to take antibiotics if you can avoid it

neuromd2012 :

and many of the antihistamines don't work well in breastfeeding either

neuromd2012 :

so an ENT doc would use a endoscope and see if there is a reason that can be fixed

Customer:

yes I don't want to use antibiotics unless they're necessary. what'd be my best option in your opinion?

neuromd2012 :

well your medication choices are really limited

neuromd2012 :

you can use steroid nasal sprays

neuromd2012 :

because they do not absorb systemically, they just stay in your nose

neuromd2012 :

and you can use saline nasal sprays,

neuromd2012 :

certain antihistamines are better than others

neuromd2012 :

you want to stay away from diphenhydramine (benedryl)

neuromd2012 :

but meds like claritin or allegra are ok, as they do not concentrate in breast milk

Customer:

Thanks. So should I request to see a specialist? Can having an endoscopy fix my problem?

neuromd2012 :

yes

neuromd2012 :

it has the potential to do that

neuromd2012 :

if you have chronic problems

neuromd2012 :

you can have obstructions as the cause

neuromd2012 :

and that can be repaired with an endoscope

Customer:

it's good to know. Thanks alot

neuromd2012 :

you are welcome

neuromd2012 :

good luck

Customer:

Thanks for your help
