Few questions before we discuss it further,

Since when you have this swelling?
How big is the swelling?
Any fever?swilling in the groin?
Do you have any medical disease diagnosed?

Customer: replied 6 years ago.
had it for about four months wasnt swollen and at first swelling is about 6cm long3cm wide no fever but lump bit warm to touch ido have swollen lymph node on that side in groin but i have had them for eight years now have hernia on that side as well no medical disease diagnosed

Looks like some sebaceous cyst pr Lipoamtous swelling which have superimposed infection.
Oral antibiotics like Augmentin for 5-7 days,Anti inflammatory pain killers like Advil,Diclofenac Sodium will help to reduce the size of the swelling as well.

When the inflammation subsided, surgical excision is the next treatment which is definitely required.During infection we usually avoid surgical intervention unless there is abscess or pus collection inside.
In case you feel the swelling is fluctuant or increasing in size, there is fever,chills, it might need intravenous antibiotics, surgical incision to drain the collection.
