On routine exam urinalysis shows 3+ WBC Esterace. Culture and sensitivity is negative. I am asymptomatic except for occasional mild right sided back pain for 4 months.  Does this need to be treated?

did you have the sonography abdomen done?
how long is the back pain and its exact location?
do you have difficulty in urination?

Gallbladder ultrasound was negative. No abdominal sonography done.
Back pain is mid back, flank, that sometimes radiated under ribs.
No difficulty in urination.
Drink minimal amount of water daily.

The back pain radiating to under ribs may be associated with renal calculi.You should have the sonography and x-ray of the kub region done to know the underlying etiology.

The other causes are colitis, cholecystitis, nerve irritation, disc prolapse, etc.The other causes may also be ruled out by having the relevant investigations done like mri of lumbosacral region, colnoscopy, blood counts, etc after consulting doctor.

What about the 3+ urinalysis and the negative c and s? Any follow up needed?

When the WBC count in urine is high, it means that there is inflammation in the urinary tract or kidneys, urinary tract stones, holding urine for too long time, etc.

You may have the IVP done to know if any small stone is present or not in the tract.The repeat c/s may be advised as well if the symptoms worsens.
