These symptoms indicate that she is having fast heart rate and this may lead to low blood pressure and can result in symptoms of hypo perfusion like shortness of breath and cold hands and feet.

As she is a know case of heart disease and has underwent bypass so there is a likely chance that she has some blocked blood vessels in the bypass vessels.

Initially she needs to get a simple test like ECG and then hotler monitoring.

These can pick up any abnormal rhythm during her normal life activities.

Nest she may need is Exercise tolerance test or stress echocardiogram test to see whether there is some angina. These tests are more than 70-90% sensitive and specific in picking up blocked heart vessels.

In the last gold standard test is coronary angiography which can give 100% diagnosis of blocked heart blood vessels.
