What does it mean to...
What does it mean to have an occult hepatitis b infection? My mother's blood work was as follows. Is it likely that she has an occult infection?
Hep B surface antigens- negative
Hep B surface antibodies- positive (She was vaccinated 3 years ago at age 53)
Hep B total core antibodies- negative
Hep B e antigen - positive
Hep B e antibodies- negative
Right now, her doctors are puzzled by her results and repeated tests and added Hep B DNA test. Could the vaccination cause the hep b antigen to be positive at this time or is it more likely that at one point she was infected with hep b?

vaccination should give hep B surface antibodies. presence of hep B e antigen indicates the person has high levels of virus and greater infectiousness. the presence of e antigen means current infection. it is strange though as she should not have both at one time unless in some transient phase. repeat tests and DNA testing is needed to confirm

How could one be infectious as indicated by the presence of hep b e antigen and also have surface antibodies? What do you mean more specifically by a transient phase?

that is why i said repeat testing is needed to confirm if the lab reports are valid. surface antibodies are formed when the body has cleared the infecton. while e antigen points to active infection. it is not an occult infection. transient phase means that the antibodies have just started to formand it may take a while to get the e antigen to be negative. repeat testing is needed to confirm.
