My son, heavy smoker for about 7 years. Quit all nicotine products for over 4 weeks. Was using commits for about 2 months before stopping. Scheduled for back surgery on Dec. 9, 2009. Needs to pass continine test by Dec. 2, 2009. Is it possible? Yes, he is exposed to second hand smoke occasionally by his friends. Buy the way, this is the 4th time this surgery has been rescheduled due to continine
Thank you. It is impressive that he has quit smoking after many years of being a heavy smoker.

Cotinine stays in urine for up to 5 days. The test with the 200ng/ml cut off is typically done with tests strips. More sensitive tests can detect it for up to 10 days.

There are things which can make this test result a false positive. These include black teas, foods like eggplant, potatoes, tomatoes, tomatillos and peppers. Some beverages can contain nicotine and he may not be aware of it.

These would cause a false positive on a sensitive test but can also cause a false positive on urine test strips.

As far as the second hand smoke: typically, exposure to second hand smoke may show cotinine levels below 100 ng/ml, or this level can be measures in very light smokers. There are two types of second-hand smoke, mainstream and sidestream - exhaled smoke and the smoke emitted from the lit cigarette. Most of the smoke in the air around smokers is sidestream, however both types contribute to positive cotinine in urine, blood and saliva.

He needs to stay away from his smoking friends for now and re-evaluate his diet - avoid the types of foods and black tea which can cause a false positive.

My question is directed to continie levels in the blood. Continine stays in the blood for a much greater time than it is in the urine. How long can contine be detected in the blood at levels greater than 200 ng/mL?

Indeed. Both nicotine and cotinine are detectable in the blood for about 7-10 days.
