Two common causes of high idle concerns and rough shifting can be caused by either a dirty and sticking IAC (idle air control ) valve and or a failing Throttle positions sensor. I would start first with removing the IAC and giving it a good cleaning and monitor the TP signal while the problem is occurring, if the signal is erratic then replace the TP sensor.
