Are you sure there aren't reminder unattended ?

Have you tried restarting the Mac ?

Intermittently also happens on iPhone and iPad

Software glitch can cause this especially since it is connected to iCloud with other devices. Try this and let me know. Go to Apple > System Preferences >iCloud and turn off Reminder. Restart your Mac and then turn it back on. This will refresh the database.

You can do the same on iPhone or ipad from Setting > account > iCloud > Reminders.
