A quick update. I found out that they never performed a reset procedure after each repair. I removed the ecm fuse for at least 10 seconds. After replacing it, had a crank no start. Performed again, engine started. Runs very rough and runs very rich. Also feels like something that would be in limp mode. Slow acceleration. Dies in flight when lifting off accelerator.

By feeling a miss you mean the engine shakes?

Was the seafoam put in the gas tank, or vacuumed into the intake?

Have you tried running it with both O2 sensors unplugged?

No i have not.... after inspecting them the look like after market wired in no pigtails exist.

OK aftermarket 02 sensors in a Toyota/Lexus is not a good thing, they can and will cause excessive fuel dump causing a misfire. I would recommend disconnecting them and see if it runs better. When there disconnected the car should run at default sensor voltage values. If they are not Denso brand I would replace then regardless.
