This notice is sent to vehicle owners to alert them when a third party (such as a creditor, or a potential claimant) has requested this information.

Usually this is associated with someone trying to enforce some sort of legal claim (so a debt collection attorney, a creditor, someone that you were in an accident with).

There should be contact information related to the claim (a phone number is ***** to be listed) on the notice itself, you can contact them directly to inquire as to why they pulled this information (they may not tell you, but you can ask).

If the information is not on the notice, contact the Department of Motor Vehicles.
