Is it legal for police to seize and remove a cell phone from a hospital where a person is hospitalized without a search warrant?

It would depend upon the facts and circumstances, but generally a person would have an expectation of privacy in his hospital room and the police would need either a warrant or an exception to the warrant requirement to make the seizure. Then, because of the recent Supreme Court ruling, the police would also need to get a warrant before they could search the contents of the phone.
