I am filing out a APP-004 form. At the very top of the form there are two blanks. I am not sure what to put in the blanks.
My case is in the Court of Appeal of the State of California in and for the Third Appellate District. Nowhere on any of my paperwork is Division mentioned.

What should I enter in the two blanks at the top that read:
Court of Appeal______Appellate District, Division_______?
