There is nothing illegal for a hospital to choose to not bill for all the procedures. Because hospitals are private institutions, it is really up to them whether or not to bill or underbill, should they choose to do so.

even when they belong to the county? It is massive underbilling.
Ultimately the billing is up to the hospital. If it is a private institution, they are free to not bill their patients if they so choose. While unprofitable and ruinous, it is not illegal.
