I'm trying to find a replace control module for my air

Customer Question

i'm trying to find a replace control module for my air handler blower motor. My air handler is a carrier FK4CNF003. The blower motor is 5SME39HL. Thanks.
JA: Did you double-check the thermostat's settings? And what about the filters?
Customer: yes. even swapped the thermostat with a functioning spare. Which filters?
JA: What are all the symptoms of the problem (error codes, flashing lights, etc.)?
Customer: blower motor won't start. Has good high voltage and control voltage. windings on motor are good.
JA: What happened just before this problem started? What troubleshooting have you tried?
Customer: blower stopped working. checked 230volts at 5 pin connector on motor. swapped thermostat with known good one. checked 24v transformer to control board. checked low voltage out of control board to motor control module.
JA: Anything else you want the HVAC tech to know before I connect you?
Customer: not really. Just need help finding parts and cross reference.
