Customer: I have a 15 yr old qh with navicular . She has had xrays 1-2 yrs ago She is now not wanting to put much wt on one side and seems worse. I would like to have her nerved and will cost around $2000 Is there any bones that have/will rotate or something else that would make this nerving not worth doing. . She is a nice well bred mare and has been used for broodmare last 5 yrs. would like to ride(light) her if possible because she is well trained . Will that be something I will be able to do or brroodmare? Will she be able to carry a foal full term? Thanks!

DrMike :

Do you recall specifically what the x-rays showed? Would it be possible to post the x-rays here?

DrMike :

What treatments have been tried since the diagnosis? Have they had some effect?

DrMike :

Is she affected in both front feet, with one more painful than the other, or is it limited to the one foot?

Rotation of the coffin bone is a concern with laminitis rather than navicular disease. The risks of the neurectomy include nerve re-growth after surgery, and further damage to the deep flexor tendon where it passes behind the navicular bone. If there are adhesions between the tendon and the bursa or bone itself, these can cause further tendon damage. Ultrasound can sometimes identify adhesions, as can an MRI.

There are other options I have used successfully in navicular horses, but if the cyst is large, the neurectomy may be the best option. It would also be best to limit riding even after neurectomy if the cyst is large, to decrease the chance of fracturing the navicular bone.

$2,000 seems to be a bit much for a neurectomy, but I suspect they may also be planning on an inferior check ligament desmotomy to decrease the tension on the deep flexor tendon.

Let me know if you have any other questions,

DrMike
