I have a 2003...

I have a 2003 odissey with 190 000 miles and have used 10 W40 motor oil but my friends say that at this time in my next oil change I should change it for a thicker one or to a synthetic ... what is your recommendation?

If you are not using oil you can keep the same oil you have now , if you would like to switch to synthetic it will let you go longer between oil changes

in the summer time a thicker oil is ok in the cold winter I use a thinner oil

once I change to synthetic I must continue to use that kind if I have to add a litter ..for example?

yes you dont want to mix them

If you run synthetic oi I would add only synthetic

Correct I would just use what you are using and keep the oil level up

I change the oil every 5000 miles

the 5 - 20 will be a little light if using oil I would use a 10-30 or 10-40
