How do I measure a bottle neck for 20/400 cap size

I need to buy a glass dropper cap for a bottle which only has a plain cap. I see many Internet advertisements for 20/400, 22/400, 24/400, 33/400 bottle caps, and glass dropper caps. So, how do I measure the bottle that I have? I have a really nice micrometer. Do I measure the I.D. of the neck, the O.D. of the neck, and if so, over the threads or in between the threads? What do these numbers mean?

Ok here is the informnation you need.

The closure or neck finish size is made up of two numbers separated by a dash. For example 24-410 or 89-400. The first number refers to the diameter in millimeters (mm).(This would be the outside diameter measured without the threads) The second number refers to the height of the closure relative to the number of thread turns on the container neck. The most common numbers are -400 (one full turn of thread) and 410 (one and a half turns). Some bottles will have a -415 neck which is two full turns
