If I had images of my brain do you have nuerologist that could look at it and say yes or no if there is a mass there that I should see someone about?

I am 34 weigh 194lbs. I always have ideal blood pressure last check at Dr it was 116/70 but I get random asthma symptoms weezing and coughing rattling in chest, been screen by a lung doctor and I do not have asthma and I have never smoked. I also get random loss of voice with no pain (currently experiencing that right now)😢 I have bern to ER and ear nose n throat Dr and have no infection, or polyps, thyroid functioning perfectly at correct size. I also get random muscle weakness in legs and arms that changes (currently experiencing that now in left hamstring) with no injuries or prior damage. Random changes in vision floaters, halos, wavy lines, sparkles, hallucinations of past images eye doc said left eye I has thin retna and maybe early glaucoma in one right eye but recently ruled it out. Not glaucoma, no diabetes no high blood pressure, or eye infection. Eye vision use to be (OD 20/10 - OS 20/20 got it tested as requirement to be a banker in 2008) it is now (OD 20/40 - OS 20/100). Random hearing loss with loud noises light fire trucks, planes flying over, smoke alarms, crowds of people clapping or cheering.

Doctor's Assistant: When was last time you had a check up? Is there a history of high blood pressure in your family?

Also have random rashes that look like goosebumps that go across face, arms and chest, get bruises out of no where. I have extreme hairloss at the op left side of my head.

I had a check up on 11/15 wit PC doc. Eye exam on 3/12 and 3/19, went to ER for on 2/14 was treated for bladder/ kidney infection (I get this alot). Was admitted in hospital and stayed a week 10/15/17 for an infection. They thought it was appendecitis, or spine infection, menegitis. All test came back normal. The did find bulging on spine at L4-L5 and at L5-S1 mild bilateral facet athropathy with no narrowing

I have Imagine with report of my spine and abdomin but it is all on a disc. My MRI of my head has no report because it was done for free as part of compensation because I partcipated in a medical study. It is 3 years old but all I have it on disc as well. How do I get them to you?

I cannot do a live phone call cause my voice went out this morning. I cannot talk past a whisper right now. This happens all the time. I am classical vocally trained and have thoughrough training in how to speak properly withough straining vocal chords. There are no polyps, infection or any problem that should be causing this. There is no pain or sorness. I gaven't been yelling or singing or anything voice just goes away randomly sometimes for hours sometimes days.

No problem with the phone call (that's something that the website automatically tries to sell).

If you are computer savvy, there is a way you can download your DICOM images to dropbox (or another file sharing website) that I can then upload and evaluate.

Please can someone who is knowledgeable about the head/brain/eye/ and things please look and the images and tell me if it looks like a mass or something is there. Please help your answers are greatly appreciated. I have other images of my spine/ lungs/ and abdomen also.

Welcome to JA. I see that you have requested a new expert so ill quickly answer this question for you.

Actually, the images that you have attached are not complete, they do show the brain but there are certain images missing specifically the T2 weighted images, the FLAIR series of images along with the complete contrast images. However, even though the MRI images are not complete, the images which are present here are completely normal and do not show any masses or any lesions of any sort. The symptoms that you have described also do not fit in with a brain disorder (the loss of voice), if you lost the ability to form words or understand speech or could not form normal words then it could have been related to the brain, but the inability to speak (but retaining the ability to understand and form words) usually indicates a disorder with the vocal cords or the nerves supplying the vocal cords.

The random muscle weakness or numbness can be related to the brain, in that muscle weakness, numbness, twitching of the muscles, blurred vision, intermittent dizziness can be indicative of MS, however, your old MRI (3-year-old), does not indicate any changes consistent with MS. If the symptoms have worsened over the last 3 years then we can consider getting a repeat MRI done.

But the MRI that you have attached is completely normal with no masses in the brain at all. I would also suggest getting evaluated for autoimmune conditions which could explain the random symptoms affecting various organs, this can be done with the help of simple tests like ESR, ANA, and a complete ENA profile. At the same time vasculitis can also be considered as a possibility.

Thank you. Dr Munneeb Ali. How do I go about getting evaluated for autoimmune issues. I keep getting treating like I am krazy. I can't talk right now and can' text to make an appt. I don't have any close friends or family that can help me. I am having a extremely hard time taking care of my 4 boys. Juat today started noticing voice goes away when I stand up and I get chills but no fever. I had a bladder/ kidney infection on 2/14 and er Dr perscribed cipro. When I follow up with PC Dr bacteria was still there so she perscribed Amoxicillan. I just finished that. My legs hurt when I stand up, i have not been doing any strenuos activity it is hard to walk have chills and I keep feeling tightness on left side of me head neck and ear and it feel like my head is expanding. I woke up to fluid coming out ears today. I have not been swimming or shampoed hair or showered. My voice goes away almost as soon as I stand or sit. It takes what seems forevever to come back

In order to get evaluated for autoimmune issues, you should have your doctor run the following tests to start with,

ESR

ANA

ENA profile

These blood tests would give us a good idea as to what can be the underlying cause here.

Thank you Dr Muneeb Ali. Your answers have been helpful. My eye doctor says I have an enlarged otic nerve in right eye and elogated shape and thinning retna in left eye. I can request pictures but don't gave at this time. This along with other issues made me question head. What do I say to get my Dr to order those test you mentioned?

Just ask them point blank if these symptoms can be due to autoimmune conditions and ask for the tests, most physicians would go for the tests.

Actually, I have gone through them all in detail and the only major abnormality which I can find is the presence of fibroids in the uterus, the rest of the scans are essentially normal and don't show any major abnormality.

Issues had in pain in hamstring region on both legs during movement and dizziness. Still taking anibiotic3/27/18 Hospital urgent care- Passed out while making a snack in kitchen. Had been dizzy, short of breath/weezing, pressure in left side of head, blurred and double vision with or without new glasses, fluid leaking from ears (no recent swimming, shampoo, or shower). Voice had been going in an out (notice it goes out when walking) voice improves when lying flat. Leg pain in hamstring and calves. Tightness in chest and sharp pains. Rapid strep was neg. ErDoc said heart and vitals looked fine. Nor other test or lab were performed was diagnosed as larngytis and anxiety was given LORAzepam and discharged.3/27/18
Home around 7pm- passed out again. Felt like I had lost circulation to legs and like everything waist down had went to sleep. Voice cam back after passing out then left once I was able to stand straigh up. Voice went in and out for 2 hours and once I tied a string around left leg (one that had most pain voice has stopped going out. Yay I can talk for now!����

Was diagnosed with early Glaucoma in right eye do to enlarged optic nerve and inflamation although right I is stronger visually. Left eye is elongated compatered to right and has astigmatism. Doc is not certain in glaucoma diagnoses and says he will wait on outcome from neurologist since I have no diabetes, no high bloodpressure, no history of eye infections.

Given this history and repeated loss of consciousness, i would advise that you discuss the possibility of autonomic neuropathy here. It can cause sudden loss of consciousness and numbness in the extremities as well. It can also cause rapid changes in heart rate as well as the blood pressure.

I would also advise getting a EEG done which can be helpful in this situation as it will show us if there are any electrical abnormalities in the brain.

I want to know if my issues seem like it could be
Myasthenia gravis or Lupus or some other autoimmune disorder. Do you know of a way I can get ANA, ESR & ENA test done whitout a doctors order and just pay oit of pocket for expense? Also do you know where I may be able to pay out of pocket to get an EEG done? I have an appointment with oncologist but earlist available date was May 2nd I have an appoint with a Neurolohist but earlist date is June 19th. I am getting the run around with getting treatment. What to do until then?

Hi Dr Ali
I am have been in the hospital since Monday 4/2 and I am being discharged today. I was admitted because I went to pcp barely walking with muscle pain and weakness and couldn't talk clearly. Voice was going in and out, had extreme dizziness, pressure in head, buring in back of head neck and back and balance issues and disorented. Also had lots of mucus in urine but they said it was normal. Had double vision and seizure like convulsions and cold chills in ER. They gave me 2 Tylenol. I have been eating and sleeping normally. Blood pressure has been low most recent 100/60 before that it was 110/65. They did CAT scan MRI and labs and found nothing. Had phsycological team check me and they released me with no psycological issues. However they are diagnosing me with conversion disorder saying it is psychological stress causing symptoms. However I have not had any life stressors, no dramatic changes to my life other than these health things. I was happy and fine till these health changes started. My vision is getting worse I know I have an inlarged optic nerve in my right eye because of unkown pressure and thinning retna in left but glasses I just picked up last week don't help now how can emotion cause vision loss. I have asked nurse if ANA test or any others you recommended were done they don't know and say I can't make Dr test if he didn't. I am crying ���� before all this I was planning to become a UJAM & Zumba instuctor. I need my body muscles and voice working for both. What should I do?

You need to consult an independent neurologist, the above tests do need to be done before labeling you as a case of a conversion disorder. Unfortunately without additional testing we dont know what we are treating and that is why a neurologist is necessary here. I would even consider an EMG or NCS if necessary as well.

I hope my questions aren't getting on your nerves. I just wanted to take this moment to tell you just how helpful your answers are and just how much I appreciate your time and care you put into answering me. I am crying tears of joy let me tell you why before my question. After I read your last answer while leaving the hospital I went to the chiropractor to see if getting a adjustment might help with leg pain and dizzines and bring my voice back. Well low and behold! He did an adjustment on my lower back with my leg bent and my voice came back immediately. I was actually talking to him while he was doing the adjustment and voice went from a hoarse whisper to loud n clear. My leg pain, dizziness and vertigo also improved. It was short lived and lost my voice and leg pain with speech issues returned in the car ride home but at least now I know it can't be conversion disorder. I got in the same position he had me in wich is laying on right side with top leg bent and voice came back and leg pain improved. However opposite leg began aching. I also discovered if I bend over with head down I can talk. But as soon as I raise my head I can't talk. Staying in these positions increase dizziness but allow me to talk until I move out of position. Leg weakness gets better for about 15- 20 min. Who would I need to see to fix this. Is a neurologist still the best route or ENT or general surgeon? Do you have any idea what might be causing this? Thank you again so much for helping figure out where to start with this. LoL! I went from thinking it was a brain tumor to what seems like something simple to fix.

I am glad to know that you are feeling better. In this situation you should contact a neurosurgeon actually, if the spinal cord is the issue then a neurosurgeon would help in diagnosing the problem. You should get a MRI of the spine first and then contact a neurosurgeon.
