Customer Question
I was diagnosed with juvenile...
I was diagnosed with juvenile rheumatoid arthritis when I was 11 years old. I'm now 42. My arthritis has always been active. It never went into remission. My grandmother had severe RA and was confined to a wheelchair. My mother had systemic lupus and very sadly died from complications of lupus two years ago.
I have been monitored for other autoimmune diseases since I was a child. Everything has always been negative except for a positive rheumatoid factor and elevated sed rate. This winter, I started feeling much worse than usual. My pain was worse than usual but more than that, I had extreme fatigue. I've always had fatigue but this was so severe I couldn't even raise my arms to wash my hair.
I saw my rheumatologist and my lab work came back with a positive ANA for the first time ever. My organ functions were good so my doctor did not diagnosing me with lupus. She put me on plaquenil as a preventative medication because she said there is some kind of immune reaction going on in my body. She also put me on methotrexate.
Considering my mother died of lupus,, I'm very concerned about all of a sudden having a positive ANA, especially since it happened when I wasn't feeling well. What do you think this means? What is my risk of developing lupus given my family history?

Hello,I am Dr. Aren and I will help with your query.ANA+ is seen in about 20% of the normal population.Since you have a family history of RA and Lupus,in your case I would suggest that there would be a possibility of an autoimmune disease coming up later.Very often these markers become positive very late when the disease is well advanced.Plaquenil/HydrooxyChloroQuine Sulphate(HCQS) and methotrexate are drugs used in RA and also for other autoimmune diseases.THere are eleven criteria for diagnosing Lupus, these include clinical findings and lab tests, these include a positive ANA test(seen in 97% of LUpus patients).Other test would include a test for antibodies to double strand DNA, antibodies to Cardiolipin and antibodies to Sm.

Thank you to the Physician who answered my question today. The answer was far more informative than what I got from the Physicians I saw in person for my problem.

You have been more help than you know. I seriously don't know what my sisters situation would be today if you had not gone above and beyond just answering my questions.

I have been dealing with an extremely serious health crisis for over three years, and one your physicians asked me more questions, gave me more answers and encouragement than a dozen different doctors who have been treating me!!
