I'm 48 years old I do not have any past illnesses I have elevated blood pressure but not super high I think it's 135 over 95 and this episodic headache before I orgasm has happens quite often but not all the time

This is a relatively benign condition.
But an examination by doctor needs to be taken to rule out any other factor.
There can be several factors in causing this headache.
Sexual excitement can lead to tightening of muscles around head & neck.
This can also be due to a rise of blood pressure during that period.
Analgesics, triptans & beta blockers are needed to control the pain if it occurs frequently.
