it is painful and no there is no swelling. You tell me it's an infection. Why? How? What should I do?

Ok in that case, there is likely bacterial infection --comes from the skin.

Sometimes can cause infection like staph or strep.

In that case antibiotic will be required.

Better to check in urgent care.
