There is no way to differentiate from urine test strip.

For that you need urinalysis with microscopic examination.

Pessary related bleeding will show intact red blood cells.

Kidney related problem will cause cast formation and also dysmorphic red cells.

The doctor has opted out. Is there something about his answer that you don't understand? A strip just indicates blood. There is no way to tell the source of the blood from the strip. That requires urinalysis.
