With a prior history of heart surgery, it is necessary that arrhythmia should be ruled out by an EKG. The other causes can be congestive heart failure anemia, thyroid issue, anxiety, medications etc. Any two causes can be concomitant. If he has any symptoms like palpitations, uneasiness, breathlessness etc. these also indicate an urgent evaluation.
