Associated itching,pain?

History of unprotected sex?

History of Diabetes,etc?

I am 25 years old.
Itching occurs on occasion not all the time. On penile shaft and scrotum.
Yes, unprotected sex but I've recently checked for stds/hiv and all negative.
I have herpes, I'm not sure if that affects this.
No diabetes
No other medical history
No medications I'm on

From what you describe following possibility comes to my mind.

First the patches are resistant to the topical medication and require more potent anti fungal medication prescribed orally like fluconazole.

Secondly they may be due to mixed infection and may require additional antibiotics to cure the condition.
I do think you should visit your regular doctor and they may want to prescribe oral medications.

Also 1% hydrocortisone can be helpful to reduce the itching as well.
