Occasionally (maybe once every 2 years) when I extend my right arm fully, I feel heavy pain in my right thumb. Stopping the extension of my arm stops the pain.

Hi. This sounds like a pinched radial nerve at the elbow level or it possibly could be the median nerve at the wrist. I'd suggest you have an examination by a doctor and get MRI's of the elbow and wrist to see which of these possibilities is the cause
