My EMG results show ulnar nerve neuropathy, carpal tunnel, radiculopathy, and "denervation changes in the dorsiflexors of the toes." The neurologist writes that
I do not have polyneuropathy. My primary care writes I do have polyneuropathy, neuropathy from radiculopathy, and neuropathy of unknown origin, referring to the
denervation changes in the toes. Who is correct?

Your primary care doctor is correct - denervations changes are typically seen in neuropathies. Your EMG has already reported that you have neuropathy, carpal tunnel radiculopathy. This indicates that you do have polyneuropathy.
