34 y/o old male throwing up, diarreah, and a fever of 102F.

34 y/o old male throwing up, diarreah, and a fever of 102F. Chills and sweats. Cold compress towels across his chest have brought the fever down but it goes back up within a couple minutes. At what point do I take him to Urgent Care? How do I know if he needs antibiotics?

It started about 7 hours ago.

The combination of diarrhea, vomiting and high fever is serious. This can lead to dehydration. There is no way to know over the internet if this is viral or bacterial infection.

If he were my patient, I would want to see him in my office or have him checked in an urgent care clinic or the emergency room.

Ideally, I think the ER may be the best option since IV fluids may be needed as part of the treatment.
