The AFP level high means there is some risk of testicular cancer.
Although infection can also cause high AFP, and since the level was measured before antibiotics, AFP test should be repeated now.
A decreasing level is reassuring meaning infection is the likely cause.
And you will also need a testicular ultrasound to evaluate the swelling better.
That means, you need to see your urologist and set up these follow up tests.
