Does the widower have any other sexual issues, or is his visible problem only the lack of semen?

The usual cause of aspermia is a blockage of the ejaculatory duct, prostate surgery, certain drugs and retrograde ejaculation.

To determine if the aspermia was from psychological issues (such as guilt) he would first need to be evaluated by a urologist. If there were no physical causes found, then it is indeed possible that a psychological problem could be a factor. The mind has a very strong effect on the body, and although it would be rare, it could be possible. But it would not be wise to assume this until an evaluation by a urologist was done.

Of course, he could have guilt regardless of the aspermia, but this sort of issue would more typically result in impotence or difficulty maintaining erection, or lack of desire. But even those symptoms can be caused by physical problems and need to be evaluated by a physician.
