Yes, a four year old can have appendicitis.
In children her age, vomiting is usually present before abdominal pain.
Fever is almost always present as well.
Loss of appetite is common
The abdominal pain can start around the belly button area but then moves down to the right lower abdomen.

Tenderness in the right lower abdomen is also suggestive of appendicitis.
Kids usually prefer to lay still when they have appendicitis.

If your daughter has a fever and abdominal pain today, I would take her to see her doctor right away to check for appendicitis.
Viral stomach bugs and urinary tract infections are other potential causes of her symptoms.
