Hello,
   This term is used commonly to describe the lesion of the soft cartilagenous structure known as glenoid labrum surrounding the shoulder joint .Mid fraying & blunting is seen in grade 1 lesion of labrum where there is loss of early smoothness & round contour with mild focal disruption of fibres due to trauma or degeneration.

It looks to be grade 1 lesion which are generally treated arthroscopically by debridement with a shaver.

Although this is is a other question then your original question but it looks that there is some mis spelling or confusion for real word(?insertional).In medical terms insertion of a muscle (infra or supra spinatus) is area where it get it's attachment.

with what i have asked do you think i would need surgery. and what is wrong with my shoulder

Surgery will be better for your early recovery although you may have treatment with conservative methods where you have to give rest to your shoulder which is followed by exercises from a experienced physiotherapist.It may take 4-6 weeks for recovery while after surgery you may start movement after the surgery when your pain allows.
