ASCVD - Atherosclerotic Cardio-Vascular disease.

It is a disease of blood vessels which become narrow due to deposition of fat inside the vessels (Old age, smoking) and when they affect the vessels of the heart, it is called ASCVD.

CHF - Congestive Heart failure.

When the blood vessels to the heart are narrowed to the extent that it doesn't sustain the heart, the heart fails which is called as CHF.
