A stroke occurs when blood flow is blocked to the brain, such as with a clot. Unilateral paralysis (just on one side) of the diaphragm can decrease oxygen intake, but this would not lead to a stroke. Most people with only unilateral paralysis have few or no symptoms, and don't even require treatment, even though physical activity might be harder for them. It can be treatment, according to the cause, but not treating it shouldn't lead to a stroke.
