You can obtain a complete copy of your work history from the Social Security Administration. Here is the form and instructions:

You can also call Social Security at 1-800-772-1213 and they will give you the totals, but the form will give you the complete written work history.

How long will it take you get info on all of my employment history with dates?

If you call them, they may be able to give you some information tomorrow or Monday. Honestly, I don't know if they'll be open tomorrow.

Getting a certified history will take awhile - the estimate is up to four months. It's a good idea to call them in the morning and see if there is any way to expedite it. You may be able to get non-certified information faster.

I wasn't able to find a free form that you can fill out online. Social Security will accept the form if it is printed in blue or black ink.
