Hello, is the engine running smooth or rough?
Have you checked the fuel pressure?
Both the banks running lean at the same time could be from too much air or not enough fuel pressure. Since the smoke test passed, next step is to check the fuel pressure. If fuel pressure good, it could be a faulty mass air flow sensor.

Its on the low side but it is till under specification.
Do you hear any exhaust leaks?
That is normal and the leak after the doenstream oxygen sensor willnot effect it. Since no exhaust leaks, no intake leaks and fuel pressure within specs, next step is the mass air flow sensor.
Unfortunately the manufacture have not provided a manual test to check the mass air flow. It has to be checked by a scanner. I would start by visually inspecting the mass air flow filament for any contamination
Is it able to perform mass air flow check?
I have a modis and i'm sure you should be able to. There shuld be an option for special functions, component tests or component activation once you put vehicle's vin#
Thats great, did the smoke test you did before not show faulty intake gaskets?
Ok, i wish i would have known that before. Either way glad it is taken care off.
