2003 F-150: cylinder..hesitates..45 to 60 mph) and lower rpms…

I have 2003 F-150 6...
I have 2003 F-150 6 cylinder 4.2 XL Super Cab truck. 120,000 miles

The truck "hiccups" or hesitates at higher speeds (45 to 60 mph) and lower rpms (1400 to 1700 rpms), when ever going up an incline (meaning that I am cruising along on flat road and then come to a rise in the road) and power is needed to maintain speed. Once power (more gas) is given, the hiccup stops.

There is never a problem of hiccup or hesitation when the truck is over 2000 rpm.

The work I have had done to date
* new fuel filter
* clean mass air flow valve
* confirmed pcv is still drawing a vacuum
* new fuel pump
* manifold gaskets (upper and lower)
* change oil and change oil filter religiously
* change air filter on schedule
* transmission fluid changed at 70,000 miles

To me, it appears fuel and "closed loop" (no vacuum leaks) are ruled out. Can it be a pcv though it is still pulling a vacuum. Any other ideas?

Verified
Sounds alot like a bad ignition coil pack, wire or spark plug. The ignition coil is a weak link for the 4.2. Try spraying water on it and seeing if it won't break up, just lightly power braking it ( holding foot on brake and applying the gas while in gear to load the engine without moving.
