By failed surgery, what exactly do you mean??? I assume you mean poor vision after surgery? or do you mean list complications that can occur?

reasons for poor vision some years after surgery
Expert:  Dr. Stevens replied 8 years ago.

Very good.....this is assuming the vision was OK after surgery and went bad years later

#1 cause - PCO- posterior capsular opacity - scar behind the implant that reduces the vision - simple laser procedure in office fixes issue

#2 - some retinal problem - most common ones are macular degeneration and cystoid macular edema, diabetes related bleeding, artery and vein occlusions (due to diabetes, hypertension)

#3 - advancing or undetected glaucoma

#4 - cornea decompensation (rare) - usually has pain

#5 - implant dislocation - super rare unless patient had severe trauma near eye

I hope this helps your query, if not let me know, Dr. Stevens
