The timeline is a bit confusing. Has the title company underwriting dept reviewed the trust and its amendments?

There is not really any such thing as a "substitute" trustee. There is a trustee, or a co-trustee (both with present authority)-or a successor trustee (with conditional authority).
Obviously, I have not read the trust and its amendments or spoken with the title company. As a practical matter, very little gets past the underwriting department and their job is to make sure the title that is transferred is insurable.
The title company has, presumably, issued an insurance policy on the title that was transferred and that makes them liable for any errors in the transfer of title. So, your sister need not worry. If there were a problem, the title company would be the ones paying to correct things.
In all likelihood, the conveyance is fine and the deed valid. If not, the title company would deal with it.
