If you had a bank account with right of survivorship, immediately upon her taking her last breath she no longer owned the account so medicaid cannot touch that account, it belonged to you as soon as she died.
As far as the insurance, they will not take the insurance, it was with a named beneficiary and the beneficiary gets that without probate so because it is not a part of your mom's estate, they have no way to take those assets.
