my mother gave me money, not a check to me but cash and a check to ford and credit card lunches on her account. she is now in assisted living and they are taking her remaining money to pay. when that runs out do I need to pay this money back to my mom's account before medicade kicks in?

Medicaid will look at every transaction over that five-year period to determine whether or not money was given away. If money was given away, Medicaid will assess a penalty period during which it will not pay for care. So, there is no requirement to pay it back to mom's account; but it may delay when medicare kicks in.
