After Listing my assets/property in the livng trust and have it witnssd and notorized, how to I transfer these assets into the trust? Do I need a separate beneficiary form?

If real estate for example, would deed it over to the trust. Personal property would just be identified, generally or specifically in the trust. Kindly rate me positively.
