Apart from thyroid, you should have other hormonal estimation too like prolactin, testosterone, estradiol, insulin levels. There are no classical signs of thyroid. So just on the basis of your symptoms; it is too difficult to predict the diagnosis. Thus only blood work up, can give you the concrete clues to the diagnosis.

Please feel free for your follow up questions.

I would be happy to assist you further, if you need any more information.

An high TSH point to hypothyroidism (low thyroid). This can explain your weight gain and BMI. Your doctor may determine by values of free T3 and free T4 to start a thyroid supplement.

But since you have gynecomastia, other hormonal assessment too would be advised. Thyroid abnormality is unusual to cause gynecomastia. A low testosterone or high estradiol or both can be responsible for it and contribute to your overall symptoms.

It is privilege assisting you.
