The symptoms you describe are suggestive of Diabetic Neuropathy.

Diabetic neuropathy is a type of nerve damage that can occur in patients suffering from diabetes.

Diabetic neuropathy can be painful, disabling.

You can often prevent diabetic neuropathy or slow its progress with tight blood sugar control and a healthy lifestyle.

The line of treatment may include

Pregabalin (Lyrica)

Anti depressant medications.

Vitamin B1,B6,B12 supplements can be considered.

You should work with your primary care doctor for diabetes management.

HbA1C testing be done in your case every three months.

Also testing your uric acid levels, lipid profile should be considered apart from free and total testosterone levels to rule out low male hormone levels.

I hope this helps
