At this point, you do need to hire an attorney, this is considered actually harassment and constitutes bad faith on the part of the insurer and you can actually seek additional damages from the insurer for them breaching their contract with you in bad faith. You need to send them a letter NOW telling them that you have been through this twice and each time their own independent doctors have found that you are qualified and they keep trying to terminate your benefits or picking new doctors to send you to and such conduct is bad faith and a willful and malicious breach of the contract with you and if they continue to harass you in this manner you will be filing a complaint with the state insurance commissioner and will be suing not only for breach of contract and bad faith, but also for this harassing conduct being an intentional infliction of emotional distress.
Generally, if you hit all of those buzz words in your letter, most times they will back off.
You are not required to report the severance package to CIGNA, it is not their business. Also, the counseling not related to your disability is not the business of CIGNA.

These insurance companies do whatever they can to not pay and some like the one you are dealing with has one of the worst reputations for not wanting to pay legitimate claims.
