Hello and welcome to Just Answer. My name is ***** ***** I will be happy to assist you with your electrical question.

1) How was the receptacle secured into a box and yet the box was covered by a blank wall plate?

2) Most likely a previous owner abandoned the receptacle for whatever the reason.
