I understand the concerns you have - very real concerns. Your veterinarian is correct and it appears that you have a very good understanding of the disease process. That said, laxis (fursoemide) the diuretic will most likely not help the overall disease process if related to HSA (hemangiosarcoma) in this case. In fact, diuretics can be contraindicated as it may lead to hypotension, azotemia (BUN / Creatitine) elevations, and weakness.
The pericardialcentesis (needle placed through the pericardial sac) is the only real way to relieve the pressure and the cardiac tamponade - which will result in more blood flow out of the heart. I have over 8 years of emergency work and the procedure suggested is not without risks, but if done correctly; most patients will recovery and feel much better within 30 minutes. I always us ultrasound to guide the needle and submit the fluid for analysis. Over the years, I have had dogs and cats have multiple pericardiocentesis procedures without problem or complication - some were needed on ER basis, others were need to maintain quality of life until the family was prepared. If I were in your situation, I would have a frank discussion with your veterinarian and make sure they will be using an ultrasound to guide the needle. We can also used mild sedation to help maintain comfort and keep Spur still - mitigating most complications.
Also - I'd recommend keeping Spur for a few hours post procedure to monitor and make sure he is feeling better.
