Hello & welcome, I am Dr. B, a licensed veterinarian and I would like to help you with your wee one today.

How old is Pixie?

ow long has she had these signs?

Do you mean her eyes are flickering back and forth?

Any chance of head trauma or access to toxins?

Any head tilting, head pressing into things, or circling in one direction?

Any seizures?

Does she seem nauseous?

She seems to be able to walk better so far this morning but still shaky

Hi again,

Now I have to say that I am concerned about your lass. This is because we can see these signs with a few different issues. If we can rule out toxins and head trauma, our main suspects for Pixie's signs would be vestibular disease (common in older dogs and often they don't eat well because it makes them nauseous), an inner ear infection, or it can be a feature of brain lesions (bleeds, swellings, infections, tumors). Furthermore, we can see this with underlying organ issues (ie liver, kidney, heart).

Now since she seems better this morning we can monitor her and plan to rest her. Vestibular disease often will resolve on its own but we do need to usually keep them somewhere they cannot fall/hurt themselves and if they are nauseous it can help to offer a light diet option for a few days. Examples of an easily digestible diet include cooked white rice with boiled chicken, boiled white fish, scrambled egg, or meat baby food (as long as its free from garlic or onion powder). Ideally, we want to offer this as small frequent meals to keep the stomach settled. And if she did have severe nausea, we could also treat with an antacid like Pepcid (More Info/Dose @http://www.petplace.com/article/drug-library/library/over-the-counter/famotidine-pepcid) or Zantac (More Info/Dose @ http://www.petplace.com/article/drug-library/library/over-the-counter/ranitidine-hcl-zantac). Whichever you use, we'd give this 20 minutes before food to allow absorption and of course double check with your vet before use if your wee one has any known health issues or is on any medications you haven't mentioned.

Of course, if she doesn't continue to improve and settle over the weekend, then we'd want her local vet to assess her. Ideally, we'd want a full neurological examination and bloods to help localize which system is involved here. If infection is suspect, then antibiotics can be started to clear that. As well, if her vet finds signs consistent with inflammation (or vestibular), then steroids can be used to reduce that for her and halt her signs. Of course, an organ issues detected on bloods could be treated. Though if her vet excludes those or suspects a brain lesion then we may need her referred to a neurologist for an MRI. Hopefully not, but in this case with her signs we'd want to use supportive care for Vestibular disease but have her checked for those other concerns if she doesn't settle over the weekend.

Kind regards,

Dr. B.
