Customer Question
My dog is a...

My dog is a 11 year old toy poodle and she is having signs of chronic kidney failure. I would like to know if flushing her kidney would help as our vet has not suggested the treatment yet.

Veterinarian's Assistant: I'll do all I can to help. What seems to be the problem with your dog?

Early chronic kidney failure

Veterinarian's Assistant: Where does your dog seem to hurt?

She is not hurting but she is drinking a lot of water and urine a lot

Veterinarian's Assistant: OK. No obvious pain. What is the dog's name?

Cookie

Veterinarian's Assistant: Is there anything else the Veterinarian should be aware of about Cookie?

She had an urine test and her specific gravity is below normal. It is 62 mg / DL

Whether or not Cookie is at the point at which diuresis (intensive IV therapy over a period of at least 72 hours) is indicated would be predicated upon the severity of renal insufficiency. Can you upload a copy of all of Cookie's test results to our conversation? I understand that you might not have a copy at home but Cookie's vet can give you one which you can scan into your computer and then give me the file link or you can photograph the pages and upload them by using the paperclip icon (if you can see that icon) or by using an external app such as dropbox.com/

I not only review the test results but also note which tests should have been performed but weren't. The specific gravity is posted as a decimal such as 1.001 - 1.060. It's not clear to me what 62 mg/dL refers to. It may be the BUN which is considered normal between 7 - 32 mg/dL.

Thank you. Give me a moment to take a look at the files, please...

I'm pleased to tell you that there's no indication of renal insufficiency in those test results. I don't see a complete urinalysis, however, which is essential when evaluating kidney health.

Thanks. Even though the specific gravity is below average? Do you need the urine test result as well? What can I do to keep her kidney healthy?
Dog Veterinarian: Dr. Michael Salkin, Veterinarian replied 2 months ago

I don't see a specific gravity posted. That's usually incorporated in a complete urinalysis which I want to see. Please note, too, that one specific gravity doesn't tell us much because the specific gravity varies throughout a 24 hour period. So far, I don't see anything you need to do for her kidneys...not even limiting protein in her diet which is usually the initial way we address early renal insufficiency.

You're quite welcome. I can't set a follow-up in this venue so please return to our conversation - even after rating - with an update at your convenience. You can bookmark this page for ease of return. I'd be please to review a complete urinalysis should you have that performed on Cookie's urine.

The specific gravity is 1.02 but they found protein in her urine. Should I’ve worried?
Dog Veterinarian: Dr. Michael Salkin, Veterinarian replied 2 months ago

Any amount of protein found in such dilute urine can be significant but it needs to be quantified to be sure. In other words, a protein:creatinine ratio is performed on the urine which tells us more accurately if there truly is too much protein being lost through the urine.

Thx. So in other words, a more thorough test should be performed but there are signs of early kidney failure?
Dog Veterinarian: Dr. Michael Salkin, Veterinarian replied 2 months ago

That test will tell us if there's early signs of kidney insufficiency. "Failure" shouldn't be a term used when everything else looks so nicely normal.
