I'm sorry to hear of this with Kilo. The aimless pacing you're seeing most often is a clinical sign of encephalopathy (brain disorder) involving the forebrain - cerebrum and thalamus. Circling and confusion may also be seen in dogs such as he. These brain disorders can have an intracranial (within the skull) etiology such as brain tumor - an important consideration in his breed and at his age - as well as extracranial (outside the skull) etiologies such as a poorly functioning liver intoxicating the brain (hepatotoxicity) or poorly functioning kidneys intoxicating the brain (uremic encephalopathy). Kilo's vet will want to perform a thorough physical and neurologic exam including diagnostics in the form of blood and urine tests in an attempt to identify an extracranial etiology that might be addressed in a conservative manner. Cerebrospinal fluid tap and analysis and MRI are diagnostics used to identify intracranial etiologies. Please respond with further questions or concerns if you wish.

I want to add that this dog used to weigh 80 lbs. He's down to 55 or 60. Even his head lost weight.

Thank you. Such severe cachexia (wasting) is most consistent with neoplasia (cancer) or degenerative conditions such as either kidney or liver insufficiency. I regret that my state board of veterinary examiners doesn't allow me to speak to customers by phone in this venue but other experts in this category may be able to assist you in this regard. Please let me know if you'd like another expert to do so and I'll opt out of this conversation. Please stay in the conversation if you wish.
